title: using table in flow layout mode

start a new web site.

Layout
 insert table
   5 rows, 2 columns

in column 1:
Name:
Address:
city:
State:
Zip:

then add 5 text boxes in column 2:

then right justify the words in column 1.
  Note: you don't have to highlight the word,
   just put your cursor in the table box
    and click the right justify icon.

then add 2 more empty rows to your table
at the bottom
by
 put the  cursor in the left box at the last row,
 layout 
  insert
   row below

then highlight the bottom 2 boxes
 layout
  merge cells
then highlight the 2nd the bottom boxes
 layout
  merge cells

In the bottom
 put a text box that fills both columns
In the 2nd to last,
 insert this text:
enter your favorite saying:

Don't worry if your text boxes have yellow
or white backgrounds.

Above the text: enter your favorite saying:
insert a blank row
 insert 2 spaces into the left
  else an empty row will not take up space.
